The 27-year-old Argentinean winger has returned to New York Stadium where he spent six weeks on loan last season, penning what is believed to be a one-year deal following his release from Middlesbrough.
Ledesma featured seven times for the Millers last term, scoring one goal, before being recalled by Boro and he spent the latter part of the campaign on loan with Brighton.
"It goes without saying that I am delighted to bring someone with the ability of Manu back to the Millers," boss Steve Evans said.
"Every Rotherham United fan will be delighted that this young man has joined us on a permanent basis as they have, like me, watched many outstanding performances when he was on loan with us last season.
"We as a football club tried as hard as we could to convince Manu's parent club to do a permanent transfer last season but sadly they couldn't sanction one at the time and it shows our perseverance has paid off.
"I got home with a huge smile on my face having secured Manu's signature in the knowledge that there was significant interest from some of our local rivals in the Championship."
Ledesma's arrival ends a lengthy pursuit by Evans, who saw a cash bid rejected for him last January, while there have been prolonged negotiations this summer, with the Argentinean reported to have rejected the Millers' first offer.
He joins fellow new signings Aidy White, Lewis Buxton and Joe Mattock at the club.
