The world number one had been struggling through out, stopping to seek attention for an issue which he later confirmed to be in his right eye and related to a pre-existing eye infection.
He recorded 18 unforced errors to Lopez's 14, and struggled against his opponent's serve.
After Lopez wrapped up the first set 6-3, Djokovic retired from the match and the event.
It is not the first time Djokovic has reported eye problems; he has previously had issues with his contact lenses causing irritation.
Lopez now faces Marcos Baghdatis in the semi-final.
