The 33-year-old midfielder, who has not played since injuring his Achilles 14 months ago, has a contract until the summer.
However, his return to the team has been constantly put back and a comeback next month was ruled out last week when he had a ninth operation.
It means the Spaniard is running out of time to prove he deserves a new deal before the end of the season.
Arsenal manager Arsene Wenger said: “I sent a message to Santi to wish him well after the surgery and he said, ‘Look, I have to start again from zero’.
It is very sad. What this guy’s gone through is unbelievable and now he has to start again.
“This is a real pain, it’s terrible news — I was hopeful that he’d come back in January but that was before this setback."
Cazorla suffered complications with his injury after it became infected and he nearly had to have his foot amputated. Wenger says the outlook is not good.
“He has to wait until he can run again, after that it will be at least another six weeks.
“I just hope that he will play at some stage this season.”
