Napoli, already sure of a second-place finish in Serie A behind champions Juventus, went ahead four minutes after the re-start when Brazilian midfielder Allan curled the ball into the net from 12 metres after his first shot was charged down.
Sergio Floccari thought he had equalised for SPAL only to see the goal chalked off for offside following a VAR review, but they finally leveled with an Andrea Petagna penalty six minutes from time.
The game was not over, however, as Mario Rui played a one-two with Jose Callejon before scoring with a rasping shot in the 88th minute. Napoli have 76 points from 36 games, 13 behind Juventus, while SPAL are a comfortable eleventh with 42.
At the bottom of the 20-team table, where the last three go down, 18th-placed Empoli pulled themselves within one point of Genoa, the team immediately above the danger zone, after a 2-1 win at Sampdoria, their first away win of the season.
Diego Farias flicked the ball in after 56 minutes for the visitors and Giovanni Di Lorenzo added the second from a rebound after Samp goalkeeper Emil Audero saved Francesco Caputo's penalty in the 75th minute.
The 36-year-old Fabio Quagliarella, Serie A's leading scorer, pulled one back for Sampdoria with a stoppage time penalty, his 26th goal of the season.
Udinese climbed above Genoa to 16th with a 3-1 win at doomed Frosinone. Stefano Okaka put them ahead with an individual goal in the 12th minute, Samir headed in Rodrigo De Paul's free kick and Okaka added a third before halftime.
Federico Dionisi pulled one back five minutes from the end, although a torrential downpour contributed to the goal as his header did not bounce on the soaked pitch, catching out goalkeeper Juan Musso.
Udinese have 37 points alongside Bologna, Genoa are on 36 and Empoli 35. Frosinone and Chievo have already been relegated.
