FULL-TIME. That's it, and England are into the finals of the Nations League!
Four minutes to be added on here.
GOAL! HARRY KANE DOES IT! Everyone promptly jumps on him. What a great ball into the box from a free kick courtesy of Chilwell, and Kane stretches his legs to push the ball home with his studs.
OFF THE LINE! Croatia want to restore their advantage and it's off the line only as far as Brekalo, whose shot is saved by Pickford.

And Rog is the last substitute of the afternoon.
GOAL! Long throw, Kane fumbles at the far post, and Lingard makes sure that goes over the line.
It's blocked and out for a throw.
England break and win a corner, to be taken by Sancho.
Dier's caught in possession and it eventually comes to the toes of Vlasic, who takes a moment to think and then whack it into a forest of legs.
Jadon Sancho and Jesse Lingard come on for Delph and Rashford.
Jedvaj gets booked, as does Brozovic, as they try to contain Rashford.
So if England score two goals in the final 25 minutes and don't concede again, they'll avoid relegation. And they'll progress to the next stage.
Dele is on for Barkley.
Croatia are loving all the space they have on the counter now, as Perisic sends a ball wide of the far post.
Croatia are resurgent. Brozovic can't capitalise on the end of a move triggered by Barkley losing the ball in midfield.
GOAL! So much time for Kramaric! There are six men around him as he turns and gets himself into the optimum position before letting his shot fly, and it deflects into the net off Dier.
CLOSE! Delph does beautifuly, coming in from the left side of the box but just putting too much fizz on it, taking it wide of the post rather than over the keeper's head.
The set piece itself is poor, but Barkley's curling effort of a cross in the second phase is quite tempting.
Modric and Sterling hit each other's shins. Free kick to England.
On the break, Sterling is on the left but refuses to hit the ball first time, and the milliseconds tick by, giving the goalkeeper the chance to position himself.
A poor corner ensues.
Perisic looks cross with himself as his effort to square the ball in the box is clipped out by Gomez.
Up the other end, Rashford runs into the box, then out again, then in again.
Milic swings a ball into the box from the left but Stones gets that away.
Croatia look like they're trying to steady the ship a little with the rejigged line-up for the second half.
England get us under way.
Brekalo replaces Rebic at half-time.
HALF-TIME. England had by far the better chances and will regret their lack of conversion. Croatia have had a couple of opportunities, notably after a loose back pass, but their radar is currently way off. All to play for.
England are passing the ball around nicely. One minute of added time here.
Another poor effort from Rebic, high over the bar again.
Rebic allows a wry smile to cross his face after shooting high and wide from an angle.
CLOSE! Side netting for Barkley on the half-volley, and inevitably one side of the ground think that's in.
Jedvaj looks slightly despondent at being asked to chase some very off-radar passes.
England are playing the ball in pinpoint triangles in their own penalty area.
Modric is livid with himself for knocking a long ball off target down the right. Croatia really haven't had that much possession yet.
Vrsaljko is replaced by Milic in an early change.
Booking for Barkley for a foul on Modric.
The ball ends up at Chilwell's feet on the edge of the box, and his shot takes a deflection that requires Kalinic to bat it away.
The crowd shout for a penalty after Kane is nudged away from the ball in the box, but it's just a corner.
Rashford does so well to hold the ball up after Pickford clears a Croatia corner, but he's robbed by an outstretched boot from Vrsaljko.
Gareth Southgate is calm on the touchline, offering gentle encouragement to his team.
Modric ushers the ball out of play under threat from Rashford.
Chilwell's set pieces are not doing particularly well so far today.
SO MANY CHANCES! Another long ball chipped to Sterling, who gets laid out by Kalinic on the edge of the box, the ball rebounds to Kane, and the keeper is on his feet again ready to get that away too.
Interesting to see Croatia quick to close down Pickford whenever he's handling the ball now.
Stones flicks the corner down to the far post, and Kane can't connect with a half-volley.
Sterling is in the clear down the left, and checks to see where Rashford is - in the middle. He opts to shoot, but the goalkeeper gets that away.
Delph hits a long straight ball forward in search of Sterling, who has his back to goal.
The second one swings in low but away from the goal, which seems to confuse everyone.
Chilwell's corner doesn't get past the first man.
Sterling does really well to hold off the Croatia defenders, set up by Barclay, but opts not to shoot with his left foot. He gets scolded by Delph.
Rashford on the charge down the left, showing off some pace and cutting in to play in Harry Kane, who's flagged offside.
What an odd chance there for Rebic, hitting an optimistic shot high after Pickford comes out to close down a back pass towards an open goal.
Kane does a backward roll after backing into Dejan Lovren as they challenge for a high ball.
England get their first touch with a throw-in.
Croatia kick us off.
Time for the national anthems.
Harry Kane lines his troops up, the Croatia team exchange embraces as they ready themselves to head out on to the pitch.
Gareth Southgate explains that although Jordan Henderson wanted to play, he's not quite ready, so he's picked Fabian Delph instead to do a job.
It's a chilly but sunny early afternoon in north-west London as the Wembley stands begin to fill.
TEAMS: England - Pickford, Walker, Gomez, Stones, Chilwell, Barkley, Dier, Delph, Sterling, Kane, Rashford. /// Croatia - Kalinic, Vrsaljko, Lovren, Vida, Jedvaj, Modric, Brozovic, Vlasic, Kramaric, Rebic, Perisic.
