After the opening runs Geisenberger, who missed out on a medal at the Luge World Championships in Innsbruck, looked set for victory as she set a new track record of 46.795 seconds.
But two-time European champion Ivanova had other ideas and she lowered that best to 46.792 in her second run to come home in 1:33.648 minutes, 0.046 ahead of Geisenberger.
However, although she was beaten to top spot three-time World champion Geisenberger still took home her fifth World Cup title as she increased her lead in the overall standings to 102 points with 882, meaning that she cannot be caught in the season finale in Altenberg.
Fellow German Tatjana Huefner, the reigning world champion, finished fourth but did set a new start record of 4.304 seconds in her second run.
And completing the podium in PyeongChang was German Julia Tabitz who could celebrate her best-career result to date.
With her fifth overall World Cup win Geisenberger has matched the success of record-breaking winners Silke Kraushaar-Pielach and Huefner, equalling their record of five overall wins each.
Meanwhile, in the overall standings, Ivanova now has 623 points and is third behind Huefner on 780.
