Stoch had already claimed the overall World Cup title in Vikersund last weekend and secured yet another triumph in the final event of the season – the 31st World Cup win of his career.
The Pole laid the foundation with the longest jump of the day, 245 metres, in the first round to earn 237.9 points and followed that up with a leap of 234.5m to finish with a total of 455.6.
That was more than enough to push Austria's Stefan Kraft into second place, with the bronze medal going to Daniel Andre Tande of Norway.
That completed a brilliant campaign for Stoch as the 30-year-old also won the first edition of the Planica 7, the Four Hills Tournament and Olympic gold in Pyeongchang.
"It was a great season for myself and the entire team," said Stoch. "I want to thank every member of our team, without them I wouldn't be here right now.
"And I also want to thank my wife, my parents, and the fans. We have to best fans in the world."
Germany's Richard Freitag finished second in the overall World Cup standings, a full 373 points behind Stoch, with Tande coming third.
Norway comfortably claimed the Nation's Cup ahead of Germany and Poland, while fifth place in Planica for Andreas Stjernen proved just enough for him to take the overall win in the Ski Flying World Cup, seven points ahead of Norwegian teammate Robert Johansson and Stoch.
