The 19-time Grand Slam champion has never lost at this stage in New York, and he now faces a one-set shoot-out to avoid that unwanted statistic.
It is the first time in Federer's career that he has gone to five sets in his opening two matches of a Grand Slam.
Federer had taken a set lead after just 27 minutes, but he went on to lose the second on a tie-break after failing to serve it out at 5-4.
Youzhny carried on that momentum into the third set, breaking in the fifth game before seeing out the remainder unscathed.
In the fourth set, Federer managed to break Youzhny at 3-1, and though the Russian appeared to be struggling with his fitness, he broke back when the Swiss was serving for the set at 5-3.
Federer rallied in the next game however, bringing up set point on the Youzhny serve, and though he failed to convert the first opportunity, the five-time US Open winner made no mistake when presented with a second chance.
