Ince, 26, drops down from the Premier League to the Championship as Stoke bid to return to the top flight at the first time of asking.
Huddersfield receive £10m up front with add ons taking the fee to a possible £12m.
“Incey has got a lot of game time for us in the last season and played a big role in helping us to achieve our aim of staying in the Premier League," said Huddersfield manager David Wagner.
“The value of this deal made sense for us. Tom is a good player, but I’m comfortable that we have good options in the wide positions.
“We would like to thank Tom for all his efforts and hard work in the last season and wish him the very best for the future.”
Ince - who started his career at Liverpool before representing Notts County, Blackpool, Crystal Palace, Hull City and Nottingham Forest - scored twice in 33 outings for Huddersfield last term as they avoided the drop.
“I’ve had the fortune to work with Tom before and he’s a terrifically talented player," said Stoke manager Gary Rowett.
“He’s very strong technically and can play anywhere across the front three, but he’s particularly good from the right-hand side.
“His record in the Championship speaks for itself and we’re really fortunate to have got him in the building.”
